Total Laparoscopic hysterectomy for large fibroids is still easier than that for a gross adenomyosis ! adenomyosis poses difficulties in manipulation, mobility and also tissue characteristics . Coagulation and shearing and transection of tissue requires more energy in terms of wattage and time duration, exposing the viatl organs like urinary bladder and ureter to thermal injury !
hence caution .... always perform this surgery with thorough mobilization, dissection and with optimum wattage and time duration of thermal delivery . your patient will b safe !!
